  • Follow a sequence of physical instructions (e.g., jump 3 times, turn around, touch your toes) to assess coordination and listening.
  • Draw or sort healthy vs unhealthy foods using pictures.
  • Create a daily routine chart showing healthy habits (brushing teeth, sleeping, eating fruits, exercising).
  • Observation checklist during group games to assess:
    • Shows good sportsmanship (e.g., cheers for others, follows rules)
    • Accepts winning and losing respectfully (e.g., no blaming or sulking)
  • Reflection drawing or sentence completion after a game:
    • “I felt _____ when we lost, but I ______.”
    • “A good teammate is someone who ______.”
